In case you didn't know at hundreds of locations around the map there are cubes that serve as the proxy for a furniture marker used by a random encounter NPC. These cubes have Havok physics enabled and that blocks VATS from seeing targets on the other side of them. I removed their physics and thus enabled VATS again.
