You can create it in the chemistry station (Utilities section). You only need Vault 111 suit and Kellogg's armor.
Stats are combination of both
Can be upgraded in Armor workbench
You can put additional protective equipment on legs and right arm
No requirements. Not tested but should be compatible with Vault suit or Kellogg's outfit retextures. Only male version. For version with pants check out Sole Survivor's outfit by Mind Bolt. It also has female version.
