The problem with shadows with a first-person view. When playing with a first-person view, shadows appear close to objects, which is why they are not visible. There's nothing wrong with the third-person view.
I came here to say this. I don't think it completely disables the Pip-Boy Flashlight mod because I can still hear the custom click sounds, but the flashlight overlay reverts back to vanilla. I didn't test it in power armor.

The mod works amazingly and does exactly what is advertised. However, due to the fact I cant use it with the shaped 'Metro Like" flashlight/headlamp from the Pip-Boy Flashlight mod, and instead I'm forced to use the Default style flashlight when in first person I'll have to leave this out of my load order for now.
Still gonna endorse because if you are someone who didn't use a shaped headlamp/flashlight mod before finding this then it's super cool and I have faith in this project if its possible to get further compatibility.
"Choose the same preset during fomod installation of Pip-Boy Flashlight" Could you be more specific please? For instance I do not what to choose with x2 file and it's not covered in your screenshots. I guess you flashlights 200 percent, but I don't like guessing. --- Thx for the mod!
Play around with a spot light.. this in turn as a different arc, it seems that you applied a 360° light there are different light sources available most shadow casting lights decrease performance drastically. some not so much...
I've unchecked the "Shadow spotlight" and checked the "Shadow hemisphere" so it becomes close to vanilla, I don't notice any performance drop but you can decrease those value to your likings
