This mod muffles the the player voice in 2 different ways depending on your current camera.
in first person your voice will sound very muffled and bassy
in third person your voice will sound just muffled
(check videos for examples, be sure to watch the whole thing to hear both effects)
The first person muffle can be a bit quiet but fallouts sound engine will instantly crash when loudening it even a bit through the sound effect chain. (jup...)
As for applying the effect when wearing helmets.. Unfortunately this effect is applied accross the board even when not wearing a helmet (where it would make sense) I REALLY tried to get this to work when wearing a piece of headgear or finding some sort of way to turn it on/off with a console command or something. i think it cannot be done without F4SE.
