Die 3 times = eternal death, unless you can't keep up the discipline to not cheat the mechanic. Since version 0.2 you can also exchange 3 life's for 1 perk point.

What do we need?
You'll need the MCM and its prerequisites.
So there's a MCM menu?
No, but I need it to read from/write to a file, since there is no Papyrus Util for Fallout 4. Also, there might very well be a MCM in future given this is the first PD Mod on Fallout 4 to my knowledge. Enlighten me if not. I figure there's going to be requests.
What happens if we lose our last life?
You'll find out.
Can we gain more life's or other things?
Yes, via leveling up. If you have 3 life's left, you can exchange for an extra perk point(or as many life's you can exchange). You'll need to craft a special "Perk Serum" at the chem station to exchange your life's for additional perk points. There's also a patch for SKK Combat Stalkers, that will enable a second Perk serum recipe if you carry 100 Death Stalker Tags. So you're able to gain an additional Perk for every 100th killed Death Stalker. Since version 0.2.3 you can also gain additional live by killing "prominent actors", like Kellog, Sinjin, or Eddie Winter.

Will it remove save games, or force us to leave the game on a "dead" save game?
Without someone with experience in coding a .dll plugin that would check for that, I'm afraid not. Though I wouldn't even know if a plugin can do this, as it would still need to reference an external variable to check if the player is eternally dead. I experimented with Survival Modes "exit save" feature, but that didn't quite worked out.
Where's the death video?
Fallout 4 freezes the game when I try to play a .bik video. Even the vanilla videos can't be played. I'm afraid I miss a change from Skyrim in the script logic.
Why can't we use Dragon Souls or something like that, to exchange for life's?
Well, make proposals, I do not know of any equivalent.
Can I change the number of lifes?
Yes. Modifie the .ini.
Oh, so it's also an actual Permadeath Mod?
Technically yes. Yes it is, if you set the life count to 0.
