Exploding heads and dismemberment is still somewhat realistic. Tho not from getting shot with things like .38 round however ... That's indeed nonsense.
But high caliber rounds or shotgun blast can indeed literally explode your head or tore off your limbs with well placed shots.
EDIT: There is mod here somewhere that does just that. Makes it a lot harder to explode or destroy limbs with low caliber rounds. Forgot what it was called but i seen it pop up quite some time ago.

Edit: judging by the video I guess it silences them altogether, which isn't very immersive either, but I assume it's an engine limitation.

In the fo4 pref inis some place, it can be turned off. Bethini may have it as well. I turned gore off, and it makes the game far better.
