Don't want to start the game with rad damage? Just hate fighting the same 12 radroaches every time you start a new game? Or are you trying to save time mod-testing, like I am?
Well, this should help. Sticks a little thing in the corner of Vault 111 that kills the roach assholes all at once as soon as it is "present day" (i.e. when you get thawed the second time).
