The Dreaded Rust Face Glitch That Bethesda should've addressed already.                                                                                              The Bug is from supposedly the Texture resolutions in DLCUltraHighResolution - Textures01.ba2 from the Male and Female.
So I removed the Textures for Face and Body of Male and Female.
Also removed Piper's "PiperHead_msn" just in case it was creating issues as well.
Tested she looks Fine in game afterwards.
We don't need Face and Body since we "Mod the Hell out of our Games" anyhow thanks to some wonderful Body mod Creators Right.

What Did I do...Well let's see                                                                                                                                                                                           Extracted and Re-packed the DLCUltraHighResolution - Textures01.ba2 Files for you. I removed the Face and Body Textures for Human Male and Human Female and Removed PiperHead_msn.     

Install                                                   This will be a Manual Install.                                                                                                              Locate your installation Path example: F:\Steam Games\steamapps\common\Fallout 4\Data.   
Now locate   DLCUltraHighResolution - Textures01.ba2 and copy it to a New Folder outside the Game with a Name you will Remember in case this Fix does not work for you .

Now Delete  DLCUltraHighResolution - Textures01.ba2 in the Data Folder.
Example \Steam Games\steamapps\common\Fallout 4\Data
Unzip my File  and Place DLCUltraHighResolution - Textures01.ba2 in your Data Folder.
After you have done that hit the Refresh in your Tab so it will Categorize properly.
You must have Face and Body Textures installed for both Men and Women via Mods before you hit Play.
 I use Fusion Girl and BodyTalk3 you can use whichever you like but you Must Install them or have them Installed already.
If this Works for You let me know Cause it worked for me.
I Tested it a few times to make sure by Triggering the Rust Face.
Simply placing the Original File back in Created the Issue again, then Removed and placed the Fix and Rust Face was....................Gone OH Yeah Baby...Yeah...
After Hours and Hours of trying to figure out WTF is going on and having the wife go "That looks Weird " well no shit it looks Weird it's not supposed to be that way haha..                                                       
30 years of Marriage "Always Communicate"...it helps.
And yes their is a lot of Us old Gamers still around.

I have added a Version for Gamers who do not use the Hi-Res Texture pack it's in the Download section. You are still required to have Face and Body Texture Mods for Male and Female.  
Used the same concept I did with Rust Face Fix for Hi-Res DLC.
Removed the Textures for BaseHumanMale and BaseHuman Female and repacked the File .
Follow the Instructions in the Readme. 
It contains a Readme for Install but basically same as Rust Face Fix install.
