It was brought up that the perk level requirements were too high (they were set for my Lunar Fallout overhaul). I've simply reverted the level requirements to the perks to vanilla levels.

Increased damage of all previously edited weapons by an additional 50%
Increased damage of many NPC melee attacks by 50% (This includes robot melee damage).
Made adjustments to multiple weapon mods. Mostly increasing damage from energy and radiation damage types. Reduced damage that the best mods for the disciples blade from 75% to 50%.
Ninja perk now affects one handed melee, two handed melee and unarmed weapons and only adds 25%, 50% and 100% sneak attack damage at each rank.
Big Leagues and Iron Fist perk ranks now unlock at level 1, 12, 30, 50 and 70. Rank 4 Big Leagues effect that damaged all NPCs in front of you removed for balance and friendly fire annoyance.
Version 1.4
Removed the sweep attack bonus from Big Leagues 4. Replaced it with a 20% chance to disarm opponents (level 3 gives you a 15% chance). The sweep attack was too powerful and made melee combat near friendly NPCs impossible. A 20% disarm chance is pretty high, but won't allow you to kill 3 enemies in one strike.
Fix: Apparently blitz level 2 was set to 100% chance to avoid all damage while moving instead of 20%.

Ninja
Redesigned so that the math is the same as vanilla, but there are three damage sets.
Rank 1: Your one handed sneak attacks now do 400% damage. Two handed sneak attacks now do 320%.
Rank 2: Your one handed sneak attacks now do 500% damage. Two handed sneak attacks now do 340%. Small, fast blades like the combat knife now do 600% sneak attack damage. (now requires level 18)
Rank 3: Your one handed sneak attacks now do 600% damage. Two handed sneak attacks now do 360%. Small, fast blades like the combat knife now do 800% sneak attack damage.
Due to how this works only the switchblade, combat knife and disciples blade get the top tier damage. This means any weapons added by mods will get the base one or two handed damage bonus.
Blitz (Duck and Weave)
I replaced the flat damage reduction with a percent chance to completely avoid damage. 10% at rank 1 and 20% at rank 2. The flat damage reduction worked too well since it stacked with how damage resistance works. This new effect should be a little more interesting and in line with the idea of this perk.
I realized while working on this that this perk and my custom blitz in 1.2 will not affect melee weapons added by mods. I'm still trying to make this work but for now it is base game and DLC weapons only.

Made changes to the perks below:
Ninja
Sneak attack bonuses for unarmed and one handed melee will be multiplied by 150%, 200% and 250% each rank. Sneak attack bonuses for two handed melee will be multiplied by 110%, 120% and 130%. This means the max sneak attack is 5x rather than 10x for one handed and unarmed and max 2.6x for two handed.
Percentages are multiplied onto base sneak attack, so they will stack properly with other sneak attack bonuses (was added in vanilla). Two handed weapons get much less of a buff as they are not meant to be elegant stealth weapons. A 2.6x sneak attack with a super sledge is still going to be devastating.
Blitz
I felt the best way to balance this perk was to completely change it. Blitz is renamed "Duck and Weave". Rank one reduces incoming damage by 10% while moving with a melee or unarmed weapon (you don't even have to be sprinting). Rank two reduces incoming damage by 20%.
This makes Blitz a defensive perk that encourages melee combat out of VATS. I may change the damage reduction amounts after feedback and play testing but this seems reasonable. Especially compared to the zero damage you'd take with a vanilla blitz/stealth build.
