Copy and paste the .json file in the rar archive to the following directory: Steam/steamapps/common/fallout 4/data/f4se/plugins/f4ee/presets.

If the folders do not exist, just create them yourself.

Start a new game and load the preset from the LooksMenu preset list during character creation - or use a surgery center to change to one of these presets if loading a save.
