   Do you only heal when you're not in combat?

      Yeah, this only modifies heal rate, which is disabled in combat by default. You can enable health regeneration in combat with console commands:
      player.modav 343 1
      
      1 = 100%, so any regen you have will be active in combat. 0.5 = 50%, 2 = 200%, etc.

   This was a good addition to the bobble head, I hated the fact that if you take all ranks of the medic perk it makes this bobble head useless.

      It will. I had that on the save I was testing with.
