I figured others might like my set up so I decided to upload it.

I've found my setup to feel more immersive and not all in your face. Just put the file in the DEF CONF folder.
The path is Fallout 4\Data\interface\DEF_CONF. Obviously you DEF_UI to use the preset. Also if you want the hp and ap bars on the same side make sure you rotate one of them 180 degrees. i.e if you want the ap bar on the left side rotate it 180 degrees so its depletes like the hp bar. I see too many presets where the person doesn't do that and it drives me insane.
