Update will be coming soon. Just got to iron things out a little and then I'll be providing an update which should FINALLY resolve the damage bug.
Details: So what I did was create another event on stimpak which applies instant healing at the moment that the damage event goes out and nullifies it. This effect is completely invisible in-game as you won't see the HP drop when the needle goes in anymore, but you will still hear the effect. Unfortunately this was not enough as if you HAPPENED to have the exact HP remaining that the needle would deal, you will die instantly when you use the stim. For this reason, I have made the damage conditional. The damage event will not go off if your HP is very low, thus preventing unintentional deaths.
I will be releasing this update soon, and then going back to testing. I may be able to make this even better if one of my crackpot theories is correct.
I did some further testing. My initial assumption was correct. The Medic perk is boosting the damage that the stimpack does up to around 29 on use (From 2 default.)
I'm trying to figure out how to resolve this. It seems like no matter what I set the damage magnitude to, it always does SOME damage. a magnitude of 0 does two damage. I need to either find a way to remove the damage event entirely (Which would likely involve fully reworking the mod.) Or find a way to mitigate the damage. Maybe flicking god mode on for a millisecond so you can heal but not take damage? I'll look into it.

I think that the damage bug is caused by the medic perk. The medic perk seems to be, rather stupidly imo, scripted to increase stimpack healing by a specific amount per rank. This increases Stimpack effects without actually checking which effect its increasing, so it may be that it is also adding the amount of healing to the amount of damage stims are doing. This mod functions by adding a 0 damage effect to stims, and my thought process is that Medic is adding to that unintentionally. Your workaround as a user is to just NOT TAKE the medic perk if you're using this mod.
Right now, I can't personally recommend using this mod, but I've gotten a lot of requests to release it again. I don't really play this game, or even really like it much, but I have a few ideas for a potential fix. No ETA yet.
As much as I enjoy this mod', this goes in the dumpster until the damage-bug is resolved. My stimpaks kills me at normal difficult even with 40+ health... I can't either believe that some are stating that this is realistic. Yes, needles do hurt. No, needles do not kill you unless you want to be killed.
When I go in-game and look at the stimpak in my inventory, it seems to say that it deals 30 damage to me when I use it. While the stimpak's regenerative properties negate this damage, there have been a few times that I've been in combat and using my stimpaks have actually killed me. Kinda funny to be perfectly honest, but it does become tedious to worry that your life-saving devices are now capable of killing you. I'm curious if you're still unable to replicate this damage in your game?
I actually kinda like it, I mean your stabbing yourself with a giant ass needle! It's probably gotta hurt! Plus it makes the game much more interesting! I now have to use other items that I normally would use for health until a have a good enough threshold to accept the needle!
Hello, I like this mod and I have the similar damage bug from Stimpak. For me it is only on characters who have taken the Medic perk.
An unperked Stimpak heals 30% with no strings attached, while a rank 3 perked Stimpak heals for 80% with -10 hp up front. I am uncertain about others. I mostly play on Hard and Very Hard.
Honestly, this sort of balances Stimpak in an immersive way for me and I actually quite like the "bug". But folks sure could use an optional file which just has the voice but no damage.

Uninstalled since I have died as a result of using a stimpak lmfao.
