When you mix a raider and feral ghoul you get a raider nightmare. You will find them across all the wasteland in raider's camps and hideouts. They are very fast and dangerous. Their level will depend on the player level or the Encounter Zone. The creatures of higher levels will be very dangerous. Sorry, guys, I've been watching to many horror films these days. I know these guys seem to have come out of "suffering" or "Hellraiser."

INSTALLATION
As usual, extract the .rar in your Fallout 3 data folder and check the .esp in your FOMM or in your Fallout 3 Launcher. Make sure you have installed ArchiveInvalidation or FOMM (or new meshes wont work properly)
