(revised comment as of 20 June 2010) I first said it was only essential for overseeing your .bsa's. But after I read the manual, I put FO3Edit right, and I already saw all the other essential tools being neatly ready from there...
Littleboy is basically essential if you start up more then FOMM and F3 itself. The convenience of not having to drag around the shortcuts (though I use Pstart anyway) only is additional to the astounding options it brings along.
For example, Vista users who can't access console, will find its Console Keybinder absolutely handy. Also, if F3 freezes on fullscreen, its hotkey trick is probably just as handy!
i have had The Pitt and Anchorage over a year now, and have not been able to play them because frackin Vista(fail). I recently installed 187Gurlz and in the read me was a link to this handy little utility. I ran it, clicked the necessary files and loaded my game. Everything shows up! Everything works!!!

I have been using this since I first started using mods, it's a great little tool that does it all. The only minor problem is that on every other program I use "Exit" is at the bottom of the menu so every time I go to quit Little Boy I end up opening Fallout. Otherwise its great.

i love this program. only complaint i have is that the disable intro option freezes my game, but i'm over that.
