I would just like to say I played and beat this mod for the first time the other day and I have to say this is one of if not the greatest mod I've every played, the scripting, the set pieces, the story all of these things were very enthralling. Looking forward to if you make any other mods!

UPDATE: I finally finished the whole thing. Possible spoilers, so tread lightly through this post or ignore.

For the most part I really enjoyed it. I can clearly see there was a lot of heart and effort put into this from the special effects to the scripting which a lot of it worked pretty flawlessly with the flashbacks etc. and it wasn't just for show either. There was a story to tell, and a good one at that. There were a lot of moments I loved from this especially telling you know who that he's in your world. XD

To address a complaint I had before as someone who plays with survival mods like needing food/water/sleep, this can be partially solved by stopping everything you're doing after you gather the fuses, go back restock up on EVERYTHING. Make sure you take plenty of water, food with you and maybe some colas in case you're stuck long term. The next time you're about to end up in similar circumstances, you will get a warning thankfully, it will pop up telling you to be prepared, heed that warning.

My only *real* issue now is that there are a few moments, major battles where the game can get unstable. Especially a particular part where you don't have your stuff and the quest is trying to force you to use spells. It wouldn't stop freezing/crashing. So I had to rely on just the equipment I picked up and barely survived it.

Overall, it's a great mod. It provided hours of gameplay. The ending left a haunted feeling and like there's even room for a sequel.

Please give someone permission to convert it to TTW, it was really fun when it was up on 2.9.4 I’m sure a lot of people would enjoy it on the most recent version

Early 2023 review: It's gonna be one of the most difficult reviews for Fallout 3 mods for me.
I remember playing this mod back in 2013, when I was much younger. All this magic stuff, strange spirits, hard bosses and abilities were cool for my taste back then. But I didn't really complete this quest to the end.
In 2023 I decided to re-visit this quest mod and find out - will my opinion change throughout the years? And, oh boy, for my taste now, "A Devils Wish" is ambitious, cool looking, but absolutely chaotic mess.
I understand what motives were behind this project and some parts of mod I really enjoyed even on my second playthrough. Horror sequences are pretty good (some inspiration from "F.E.A.R." and other classic survival horror titles is very obvious), some of new enemies are interesting to deal with and few locations' design felt very unique.
But as a whole quest project, the only thing in my head, after finally completing this quest, was - Is it really deserves so many praises from players? I think it's not and it really depends on your own criteria for a good quest mod this time.
Even now the mod isn't very stable with high-spec PC (I don't think that my RTX 3070 and Ryzen 7 are a problem, no?), creation engine sometimes just couldn't handle so much objects, particles and effects at one frame. And that's why late stage content can be frustrating to say the least.
And from gameplay perspective it's pure action non-stop. It's not bad for many people, I know. But after so many years my taste is really changed and I just can't mindlessly shoot another glowing guys in robes for dozens of times.
Also author clearly used too many vanilla interiors with little additions to create new cells, for example, first mod's location - Bavaria Hotel used Tenpenny Tower interiors and Fort Independence cell mixed in weird "cocktail".
I don't want to discourage someone from trying this mod, but if you need some kind of narrative and story to compliment gameplay, there's very little of it. It's style over substance in many ways and, again, more isn't always better than less.
And I clearly don't see any "great" story in "A Devil's Wish" like many other users. But maybe someone has very low standards for narrative part, huh.
Should you try this mod in 2023? I can't give you proper advice this time, because for people like me who like action, but too much of it can become boring - these people won't like this quest. But if you only want to see "cool" new abilities, shoot some weirdos and try all your gun arsenal. Well, then it's better to try it for yourself.
Endorsement from me because I still think it's a decent mod quest, but not for my preferences.
Warning! This mod will conflict with magnificent Alien Resurrection Part 1 from JW1, so I suggest to complete them separately.

Spoilers ahead to anyone who hasn't played this awesome mod.
For the lore of my character, I like to think that the personality of Prometheus alone was hiding in the section of brain that Tobar the ferryman cut out there in Point Lookout.

One of the most amazing FO3 mods I've ever seen, when it comes to visuals as well as when it comes to the gameplay. I'm just about equally amazed that in a mod this huge I haven't bumped into a single bug or technical issue, and things got much more clear and logical now that I see that this mod author is the same person who made New Vegas' fascinating "House of horrors" mod...

I'll rephrase the mod author's suggestion a bit: when you start the mod, make sure your character's level is very high if not the highest possible, and make sure you come in armed like a TANK Power armours, extreme weapons, loads of ammo - none is too strong and none is too much.

    Oh yes, just a few hopefully helpful tips;
-For those who are not big on using spells (like myself), don't worry, you can finish the whole thing by using the "regular" FO3 weapons (conventional, energy)
-If you find some situations just too hard, you can temporarily enable god mode via console (tgm)
-Don't get scared by the puzzles, some might seem tough but there's always good logic behind; if you need to cheat a little bit (like me...) you can always use console to "fly" around (tcl) to figure out a room layout, for example - but don't use it to just fly through a stage because you might mess up triggers and scripts
-Bring a companion, it can be very helpful in tough combats

TGM + The Devil's Throat EVERY time I've played. I can not see the point of dying over and over again until you get the shaft's obstructions memorized or you just get lucky. And for the simple reason you should always restart FO3 before loading your game again and most of the time I ctd when attempting to load while playing anyway which makes trying to accomplish this without cheating even more pointless. Aside from this the mod is great, the dialog is witty, the voice acting is hilarious (I know some people have criticized it though I think it's awesome in it's own way), the story is fantastic and the rewards are epic. I don't bring companions however, since it would not make sense for them to be able to join you in your own mind, which is where I figure a lot of the events are taking place. I am always better at getting through fights by hiding and sneaking around and hitting my enemy from the shadows anyway.

The Devil's Throat, iswherer I guessed you must have Gnome's Teleport abilty to jump to each obstacle.
I can't see how to survive it otherwise.
My character dies just hitting each obstacle on short falls.
Project Nevada's (FWE simulation) rebalance of damage is probably killing my character.

The game mechanics don't allow it; but maybe you could surf ride a cprpse down the shaft?

TTW has a mod conversion process using FNVEdit which I used and my character is in the mod this week.  The front door was blocked by rocks (that can be fixed if I track down the cell) but using the console you can 'zap' the rocks and find the door (my game is so heavily modded the rocks could be from some other mod.  I hit max load order and had to combine 3-6 mods into single .esp's).  If the mod has voices, they are missing, but the other sound effects are working.  Manual editing should fix the voice directory.

      SPOILERS ahead:
Playing at hardcore is kicking my ass at level 10 of 50 max using Project Nevada rebalance.
My Robco Certified robots gave me a chance to succeed until a certain warehouse when the pathing gets convoluted and the Moira companion committed suicide.
Then the next room,  Kali bramin companion, and all the Robco bots, will die without using the console moveto player commands.  I think my character was supposed to catch the teleporting gnome before she got to this area; but I never found it and that could be a scripting bug from the conversion or just my failure (I'll go back to a save and see if it's still catchable).

My roboticist character just can't beat the monsters in melee alone on hardcore.
So realistically she died in this mod.
Wished I waited another 20 levels before entering.
Guess I'm going back to the wastland and gaining another 20 levels before trying this again.

Warning: This mod has 7 NPCs equipped with the vanilla-unused item MS03SurgicalMask item, (FormID 0003E591). which FOOK2 repurposes as a scripted token used for its Refurbished DLC module (to eliminate right-out-of-the-vault DLC start and notifications; it needs to be used on a vanilla fallout.esm item).

This leads to semi-constant crashes when using the two mods together.

The solution is to remove that item from the 7 NPC templates' inventories (and optionally replace it with the xx01B519 item from FOOK2.esm, which is a clone of the mask without the scripting). Doing so completely removes the related crashes.

I may release a plugin that does so myself in the future, but if anybody else uses FOOK2 and reads this, it's very easy to do so yourself: use FO3Edit to find the 0003E591 entry, click the Referenced By tab, and in any case where it is on an NPC or placed in the world by a non-FOOK2 mod, replace it with the FOOK2 mask.

"Warning"?! Dial down the rhetoric hombre. FOOK2 is a messy, unbalanced, buggy (and beta since forever) total overhaul it's users should know they simply can't expect to be compatible out-of-the-box with many mods. And very few* gamers use FOOK2 anyway.
[*even counting all 200k unique downloads as happy keepers, which many won't be, it's still <1% of over 20m FO3 sales by 2021]

This mod is nothing short of amazing; it was clearly made with passion and OH does it fucking show, from the scripting, the locations, the boss fights, the psychic abilities, and especially the storyline. Took me 5 hours to finish the mod entirely. Though I do think the final 2 bosses were a bit anti-climactic, nonetheless, it was all worth it since every second, there was something to do.

Without a doubt, this (along with some other mods) is the best FO3 story mod I've ever played.

without a doubt, one of best mods I ever played in my life, great story and game play, the story reminds me to F.E.A.R and Dishonored both in the list of my favorite games, I recommend this mod to whoever is looking for good mods in the old games, for the creator, you done a improvise job here, I hope you doin well, looking forward to see more great mods from you, keep up the good work my friend.
