Don't Know At the top of my head but but it was a few params lines that are affected I just used the modified search to show what was green from the other mods when merging them, but yes warping or dying will reset the buff and it will need to be applied again
Somebody here did Passive HP/FP regen by adding it to the speffect param of both the right and left hand. You can go by interval with set increments, or by percentage of max hp/fp. Been using their method for a while now without any hiccups, and with multiple mods.
