The weapon you get here (Heart of Pride) is not the last one, there's one more in The Darvaarad. Still, after getting this one too it doesn't work and achievement doesn't pop :<
I was able to get this to work. I had to edit the save with HxD (followed Padme4000's tutorial on Youtube) and change the patch number to 12 (which is what my game's patch file is set to). I was then able to load the save in Inquisition and get the achievement to pop.
I just replayed the game recently, and finished it once more just yesterday, and was thinking of going back and working on the remaining achievements. I kept having loading issues and couldn't be fast enough so huge thank you for this!
