Caught another one, but as it was during a non-modded session, it's possible it's already corrected by the mod:

It's possible the mod already corrects this one, as it was something I saw in someone else's youtube playthrough, and they aren't using the mod:
In the video, their female elven Inquisitor went cross-eyed when picking up the "Letter of Confirmation" in Emprise Du Lion and spoke the line "I've found a letter. From Samson himself."

This is just a guess, but It's possible that it happens again at https://youtu.be/Qffy1Uu-2oE?t=5856 when she picks up another letter, "New Orders", and speaks the line "We should inform Cullen that we've taken care of Samson's Red Lyrium hoard." -- You can't actually see the inquisitors face, but you can see that she tilts her head slightly back when speaking that line, as if looking up.
As such I guess I found another occurrence of cross-eyed-ness!

During "Under Her Skin" quest in Vicinius' house reading the first note (that hints to Calpernia not tolerating the abuse of slaves) after entering (or closest to exit, however you look at it),
the Inquisitor says some words about the note out loud for those who cannot or don't wanna read it (I have a suspicion based on how you describe this animation's causes it's because I had no party with me, but still, I noticed the weird animation and remembered about your graceful offer =D)!
It seems Vicinius angered Calpernia by sending her mistreated slaves. She was paying a fortune for literate slaves. Why? What can slaves do that Venatori can't?
okay, well first line was already covered in 3.2, but for the last 2 lines i've made an update to include them in file version 3.3, let me know if it works.
You are amazingly fast! Sadly I am kinda rushing through the game right now, so there is no checking this moment for me. But, since the rest of the implements work, I am certain this one will as well!
During "Cleaning house" (The Storm Coast area, automatically starts upon first arrival) when reading Bandit Notes next to the dead soldiers we are looking for, the Inquisitor says: Our men were murdered by a group called the Blades of Hessarian. It seems there's a way to challenge their leader.
Non-cutscene dialogue. Crosseyed even with companions present. Can confirm both Human and Qunari are affected
Yeah, I figured as such about the dialog a long time ago, but, I don't know which dialogs are ones where the inquisitor speaks first, which is usually where the issue happens when it's out of cutscenes, so, I still take suggestions regardless, with the understanding that I'm likely not gonna find out about or make a fix for every single time it happens, but I'll do ones that I personally encounter or are pointed out to me.
Found another occurrence, but it's during a non-cutscene so it's really nitpicking:

It happened when my Female Qunari Inquisitor activated the final glyph during the Scattered Glyphs quest on the Exalted Plains, which triggers the dialogue line "That looks like all of them. I'll see what the researchers at Skyhold make of this."
Thanks! I've now addressed this line in the newest version of the frosty file that I've just uploaded, version 4, let me know if you see any more occurances, regardless of whether it's in a cutscene or not
Correction: Since I don't have that playthrough anymore, only screenshots of it, I had to watch one on Youtube, so I could tell you which one was it, when it occured to me, that I was mistaken. I'm sorry, I was wrong. Due to the face characteristics of my inquisitior it completely looks like her eyes are crossing when she looks high up, but as I looked an other character it occured to me, that it's not the case. It is the first wall carving I speak of, though.
I haven't had a chance to download or mod anything in a while, so I haven't had a chance to check the files this edits... did you happen to fix the occurence in the prologue when the Inquisitor asks Cassandra, "Where are all your soldiers?" I know it's minor but that one always drives me a little batty.
i think the idea was for it to be a fleeting-second expression and then go back to normal; the developers probably just didn't implement it well, so the inquisitor's face ended up "stuck" like that until the end of that dialogue line
that expression is what happens when a character tries to look at someone who isn't there, same thing happens if a character fails to load into a scene, but someone else still tries to talk to said missing character, so i'm guessing they set the inquisitor to make eye contact with the wrong person?
