Here is one possible set of the solutions for the floor tile puzzles found in the temple.
(There are many different paths for each, and one can complete numbers 2-4 in any order.)
Despite what some have claimed, only the controlled character can toggle the state of the tiles. This can be seen quite clearly in the 3rd and 4th puzzles when Morrigan and the rest of the Party run all over them.
