I agree with the others below its the Nose specifically that weird cartilage near the opening it's too round and looking at the model hers is more how do I say it stout and almost straight similar to that of Trish in DMC4 model. the eyes for her are a bit big compared to the model they used as reference, plus I think rather than the mouth it's her cheeks, and her jaw line that are making her look like a dude to some people, I think that's your best bet to play around with those.
Personally I think that the problems with Trish are that most of his face features are descompensate with her face scheme. Her eyes are too big, as well as his nose (which should be smaller and sharper) and mouth. Her eyebrows are too long and overall I think that hairstyle doesn't make her look good. Even with that the effort is very appreciated and I'm glad someone is working on her.

She still looks like a hag but it's not your fault dude, Trish is simply botched in this game, I don't think she's redeemable to be honest, as someone else said, Capcom just didn't give you much to work with, it's nearly impossible to fix that face, IMO.

Yeah that's what I'm trying to do....but no matter how much the model looks like her in 3ds max, once I upload it to the game it just looks soooo different.
Personally reckon should soften/flatten her pronounced facial features given the game's harsh lighting, especially her brow, the nose I was actually mostly fine with, reckon you took too much away from her nasal ridge and actually made her brow even more pronounced now that I had a closer look. Would be better if you increase the root instead to smooth out the profile I think, lift and/or reduce her nose tip too maybe? And reduce mouth width and overall jaw/chin size too perhaps. Aaaaannd if you want her to look younger add some baby fat to her cheeks. Just personal opinion tho take it or leave it, and don't overdo it lol.
I really think that your mod is an improvement to trish's face, good job! Because when I look at her in mission 10 in photo mode it's not as bad. *This is a theory, but it could also be that the voice actress's face is very unsuited for the face model that when it animates it does something weird to the model, but I actually doubt that it would produce a noticeable effect.
Okay - so seems like consensus is that people really just want the DMC 4 face. I'm gonna leave this one up - because I personally like it and for anyone who does they can download it too - but I'm also gonna work on a face rework that adheres more to DMC 4. Thanks for the feedback guys! Especially those of you who gave me specific details - they were really helpful!
