If you have DSFix, find the DATA folder and drop the image file (50151969) into the tex_override folder within. Then start the game normally.

If you have Dark Souls Drag and Drop Mod Manager (DSDAD), simply drop the image file (50151969) into the Mod Setup and hit "Launch Dark Souls w/mods.
