My version of Wulf's Dark Souls Connectivity Mod has '2016.11.12.01' in the lower right corner. It has worked in the past, like mid-2017. Don't remember where it came from, it might have been moddb.
The version available on nexusmods.com has '2016.05.31.21' in the lower right corner. This would indicate it is an earlier version.
