It is logical that a follower will abandon you if you become a vampire, but it would also be interesting if when your character is a vampire, he/she attracts the attention of other types of admirers, people who wish to become one, and offer you their services and their blood in exchange for your character converting them after a period of, for example, ten years.
It would be cool if commoners could also give some kind of bonuses according to their profession, similar to how Might&Magic 6 did it. Probably out of scope for this mod, but just throwing it out there.
Currently not possible with the quest system ... but if the Quest Actions Extension mod gets expanded with being able to apply spell effects to the player character (something that's being considered), it would in theory be possible. Unfortunately it's not possible to identify the "profession" of the commoner you are talking to in a quest so it would have to be a random and generic thing... for example, if you have a follower, you get a feather weight effect applied because they are "carrying" some of your stuff ... or a Personality boost because they are bigging you up all the time ... something like that. I want to check the basic mod is fully working first though, before I start adding stuff.
