1) You have the DEFAULT version of Unique Eyes installed.

2) You have JSILVERHVND's ICONIC UI Framework installed, if you use my custom UI icons for the eyes.

3) You don't have conflicting eye mods installed.

4) You've tried adding a ! to the file name to bump it up in the load order.
