Had some time to test in 12.3 today; everything seems to work fine, including punching.
If certain sounds aren't playing for you, it's most likely another mod overwriting this one. Mods that add custom blood particles are the usual suspect, as they overwrite the same effect files as UAO.
For the people wondering if it works for nomad: Yes! it does atleast the UAO on mod.io works on nomad its a little buggy but it works if you dont have any gore mods that change the same files
