I think I found out why this mod didn't work some people.
The item id used by the mod to check whether the ring was equipped seemed random in different saves...... For example, you start a game , made a character, the id was "AAAAA_1111111111111111" , you made another character or the game got an update ,BOOM!The id changed to "AAAAA_333333333333333" .
So this is the true cause of all these inconvenience.
Tomorrow I'll upload a new version , change the item checking to a much more soild way like those script heavy mods does.
Another heads-up that, almost two months later this is still not working for me. I can only assume there is a conflict somewhere, so I will start doing some research and report whatever I find.

So in Patch 8 currently, can confirm 100% (since I used item spawner to check) I was able to get the ring (still in act 1). However, if you're looking to use this mod dropping just the PAK file into the mod folder wasn't enough I had to use the BG3 Mod Manager and drag it from Inactive > Active and then save > export, and then launched the game. I hope all you lovely people are able to get it to work as well.
Hi, I just finished Act 3 (as in a few minutes ago) using this mod and it worked perfectly. I'm on Patch 7 (wanted to finish this playthrough before updating, in case it broke mods/saves). I have over 500 mods installed and had no issues with conflicts. As others have stated in comments here, make sure you have the ring from this mod (Forgotten Mage's Unending Resolve, which you can purchase from Helsik at the Devil's Fee) equipped before you free Orpheus. I put it on before confronting the brain in the Morphic Pool (i.e. before your tav gets sucked into the prism for the last time), just to be on the safe side.
When you talk to him the first time, choose the "anything else/something else" option. I then said I needed to think, had a chat with Gale about the whole orb thing, then spoke to Orpheus again and said I wouldn't become a mind flayer. When he said he would do it, I thought I'd done something wrong, but selected the "anything else/something else" option again. After that, he opened the portal and I jumped in, honestly expecting to see Orpheus turn into a squid when we got there, but he didn't, and neither did anyone else, so even though the dialogue made me a bit uncertain, it absolutely worked. Nobody turned tentacly and I got all my happy endings (or bittersweet in the case of Karlach).
Apologies if you're asking about whether it works in Patch 8, as I have no idea about that yet (and not sure anybody on a new playthrough could be at the final fight already, and happen to have this mod installed, considering the patch hasn't been out that long, and only a handful of total BG3 players will be using this specific mod).
As far as I can tell, no. I kept getting the "failed to load main menu" popup, and disabling this mod let me load the main menu again. It might be a conflict of mods, but this is the only one I disabled to get it working again.
Worked perfect!! All I did was put on the ring before rescuing Orpheus (breaking the shards holding him). Told him "There has to be another way", he then opened a portal. Play out everything as is, then when facing the Elder Brain, I used the passive icon ability shown on the right on the crown - waited 1 turn, then a cinematic plays of the portal opening to face the Elder Brain. I fought it - it showed only a short cinematic as me as the illithid, but other than that literally no mention of me being an illithid.
It sounds complicated, but its fairly simple. Put ring on before talking to Orpheus, say "There has to be another way". Play as usual. Get to the part where you face Emporer. Go to the crown. Use Passive ability on it (its a concentration spell, so if you get stunned, you have to do it again - but its only just for 1 turn, so you can re-do it.). Cinematic plays. And that's it! Just fight the brain!
THE only weird thing I had was Karlach's ending - my only options were to let her die, say goodbye or go with her to Avernus. I did that, and at the celebration party, my partner talks as if I never went there, so it ALL worked out perfect at the end!!!
P.S. I had a s#*! ton of mods, so Party Limit Begone didn't affect it at all, only just getting ALL the characters through the portal to defeat Elder Brain (only because I had a mod to increase difficulty for Tactician, so I needed everybody)

EDIT on (14/04/2025)/(04/14/2025): MOD STILL WORKS - Just used the passive ability on the crown before you enter to fight the elder brain - worked instantly. Same steps as I did in the first playthrough I had in 2024 with Orpheus - no change to that. The only issue is just a random illithid form you take during the cutscene of first entering to fight the elder brain, but other than that, WORKS PERFECT!!!! ALSO for my Karlach ending I had before (above) - I literally forgot to convince her throughout the game to go back to Avernus - so thats why she died. The mod wasn't doing anything
Unfortunately, it doesn't seem to work. I keep getting the message that I don't have the stones, can't pass through the portal, and will be dominated by the Netherbrain if I do.
It is related to this mod not functioning properly.
I have the stones, wear the ring, follow the dialogue with Orpheus and get the acknowledgement that someone has already done it. So, no one in the party has become a mind flayer. Yet, when I try to get through the portal the game does not recognize that it has been done. So I cannot pass through the portal without someone becoming a mind flayer.
Only after someone has transformed does the game then function as it should and let the group pass through the portal.
Just tried it and it work on for me. Patch 7, heavily modded save. All I did was equip the ring, talk to orpheus, say I'm not going to turn into a mind flayer, then choose "had to be another way". Portal open. Everyone go out. No hassle no problem.
The mod work. It might have conflicted with another mod on your end @HyenaKing
The only other mods currently aside from this one are texture mods. So see nothing that could conflict with it.
Done the same dialogue with Orpheus, get the 'Someone else has done it' response. Wear the ring and then try to pass through the portal to get the same speech from the narrator about missing something. Then when passing through the portal, I get an automatic game over as the Absolute dominates my character.
@HyenaKing Try wearing the Ring before freeing Orpheus, then talk to him. After commenting on here I did it twice. On one I wore the ring the entirety of act 3, on the other I put it on right after the dominating the brain session at the morphing pool. Patch 7
Unfortunately, this mod encountered some issues while running in my game. When I had a conversation with Orpheus, I declined the option. Karlach continued the conversation and I also declined.Then, Orpheus say:"Someone's done it already, it seems there is honor among your ranks". However, when I was about to pass through the portal, the narrator said, "You may believe you have everything you need to defeat Nether brain, but you're wrong, you're missing one thing" and kept preventing me from passing
I copy and paste " UUID:d7434a20-b3dc-4dbb-b820-e5dbbd799731" in the BG3 Script Extender, but it gives an error and the ring doesn't spawn. How exactly am I suppose to make the ring spawn?
It worked for me on patch 7. I popped the ring on before freeing orpheus, told him I needed time to think and then asked if there was any other way and he opened up the portal.
