Haven't tried this because the bug seems a serious one and Patch 5 is not exactly forgiving when it comes to adding and removing mods unlike previous patches where we can add or remove mods at will and then brute force loading saves without problems. Most of the time.
I'd suggest that instead of attaching the effect to clothing, etc., make it as a new scroll (new item ID, etc/) that will be expended upon casting and if the player is confident that it will work without problems after that, then they can either scribe it as a permanent addition to their spell book or just keep the scroll in inventory.
Also, can the player turn it on or off.? Not via putting on/taking off clothing item (as it seems from what you've done here) but as a spell option like dismissing summon or changing/ending disguise after casting Disguise Self.
I would love the eyes to be in the cutscene as well! Looks amazing on the characters but immersion breaks a bit when you talk to someone and your godly visage looks like you forgot to put your deity contacts in for a minute lol
