So far, the only solution I've found is realizing you're going to miss out on at least one of his scenes. If you have him as an avatar during the first long rest (before reaching the grove), you get Tara and the Wizard of Waterdeep quest, but lose out on his first scene about needing magical items. (Instead when it triggers, it will be the second item.) If you don't have him as an avatar during the first long rest, you lose out on Tara, but get the normal needing magical items scene.
Just completed a run as Astarion with this running and had a blast. Great mod.
The only flaw was with other characters not being able to talk to Astarion when they are avatars (I know why this is done and I have no issue with it at all)
Question though about using Wither's Wardrobe to dump your TAV in so that you can do a playthrough where everyone can talk to each other. Is there any way to get the Wardrobe when playing solo? Do I have to get someone to join multiplayer at the start, or can I start a multiplayer game without anyone else joining and fudge it that way? (if so, how?)
The ideal is to start as a custom character, then dump it in the wardrobe as soon as I can recruit Shadowheart and get Withers. That way I could run a group of 4 Origins and keep the custom locked up (using the party limit mod)
**UPDATE** Got this working :D
1. Shut Down Steam (CTRL-ALT-Delete > Task Manager > Scroll down to Steam, right-click it and stop the process)
2. Start up the Game using the bg3_dx11.exe file in the Game folder.
3. Create a CUSTOM Character - No Origins or DURGE
4. Escape the Nautiloid and recruit Shadowheart ***AND SET HER AS AVATAR***
5. Go to Game Options and set the game to the lowest resolution (you will be running 2 instances of the game now, though your PC may not need to drop the resolution)
6. While still in game, ESC>Multiplayer. Click the Multiplayer Settings cog and select LAN.
7. Tab out of the game and start another instance of it (bg3_dx11.exe).
8. When loaded, select 'MULTIPLAYER'. Click 'LAN' and select the server you created in the host and join it. You will have to select all of the addons individually to get them running and join the game (OR copy the contents of the Profile1 folder to the DEBUG_Client folder.)
9. Accept the connection on the Primary (host) game and give the Tav you created to the Debug_Client_Profile_1 account and click 'Apply'
10. Both games will now load. Once loading is fully completed on both games and you have control of Shadowheart on the main game and the Tav on the second game, ensure Shadowheart is set to Avatar again and then disconnect the secondary game.
11. Using Shadowheart on the Host, try to talk to the TAV. You should get a message stating that they are unavailable and you can send them off to Withers' Wardrobe.
12. You are now playing as Shadowheart Origin and when you recruit any of the other Origin Characters, you can swap between them and talk to each other whilst your TAVs are locked in the Wardrobe and you will never have to see them again.
I have 165 Addons running and have only had ONE issue. I have 'Really Shadowheart" and 'Really Shadowheart Extension' addons running as well as 'More Reactive Companions'. Sometimes when the characters talk, the Avatar can turn off for your current Avatar.
This definitely does still work on Patch 8. I can't speak for the Epilogue yet, but I'm fairly deep into my playthrough and so far I haven't run into any issues.
Durge has romanced Shadowheart and Astarion with no issue. Shadowheart romanced Gale as well with no issue. Karlach and Wyll are dating, as are Lae'Zel and Minthara.
If you're not opposed to lots of savescumming to see what camp event you get, you can easily witness a lot of romance scenes with the couples. As a Shadowheart Origin enjoyer who still likes having Durge around, I can play most of my game as Shadowheart with the Avatar tag just fine, and then talk to her as a companion with Durge whenever I like. This mod has single-handedly revitalized my love of BG3 and I no longer need to chose between playing Origin Shadowheart or romancing her as Durge. It's also just amazing to see unique origin dialogue. I honestly can't recommend this mod enough.
Karlach and Wyll were coupled and had couple dialogue, as did Lae'Zel and Minthara, but the game acted like Shadowheart and Gale weren't together, probably because Durge already partnered her and she acted as the main partner. Astarion still had romanced dialogue though, as if we had spent the past few months together, even if he didn't walk in with Durge like Shadowheart did. Halsin acted as normal, recognizing Durge as his long-distance partner.
Every time I rest, my characters won't wake up. SE says "COMPATIBILITY WARNING: Osiris symbol 'DB_MyNpc' referenced using incorrect case; the correct name is 'DB_MyNPC'", then I think it deletes characters. It's different characters every playthrough. When I isolate it and try to long rest only with the passive on that character, I crash
I'm slow and confused. I started a run as Astarion and i'm romancing Shadowheart.. Soon I will have the wine scene. I'd like SH to experience the first Astarion romance scene. How do I do that? Sorry for being so unimaginative. Also, does the extender configuration persist as Extender is updated?

Thank you for the detailed description, especially the part about the gotchas. Honestly i really like the concept of this mod and don't see myself playing the game 6 more times to see all the extra dialogues and scenes.
However i do care about my Tav playthrough and am somewhat worried about messing it up. At the same time i like the concept of having a companion take the lead for a while with Tav on the back...
May i ask for some extra advices on how to use this mod safely? I'd like to hear from both author and users how did they make it work without too many hiccups. Also what does happen when several characters have the passive toggled on at the same time?

If you have, how did the romances work exactly. Like if you were playing Urge, could you talk to Shadowheart about her relationship with Gale.
