Did you use the fast travel mod and travel out of a quest area? That can break a quest especially if another one is linked to the end of another. Did you leave the area and do another something else before speaking to him? When you were doing the quest "nameless" where you talk to Cravens "Skjall" dead body. It looked like you you never went down the well to find Skjall. When I went back there the clues were still in red "not found" and after I found them I jumped down the well. It pulled the quest back into the main list but Skjalls body was not there. I tried all sorts of other things. You are playing really weird in my opinion. Level 21, no witcher gear, you're not doing contracts and not grabbing them from the boards. You're not making potions, decoctions or anything really. You not buying maps, or recipes, armor or sword diagrams. You didn't even own a steel sword. Anyways, you;re save is hosed. Reload a save from much earlier or start fresh and maybe try the next difficulty up cause that was stupidly easy
EDIT: seems this is a bug for some. Patching back to 1.06 solves it for some. Then after they get it working they update their game. Read through this and you'll see a couple of possible fixes. The 2nd fix seems the easiest to try first.
tried it, but cant talk to the old man. seems it is bugged. try loading a previous save where you have to talk to the baron s men again and continue from there.
