Very underrated mod. Works perfectly after resaving in CK to give it form 44. If anyone wants my form-44 version with health warnings set at 60% and 40%, here is my google drive link to the file: (The author DavidSid has set "upload" and "modification" permission to "yes" on the oldrim page)
Personally, I would have liked too. See even why not separate the blurry vision feature to have a third threshold. Unfortunately, this comes from the vanilla structure of the game. Skyrim offers only 2 low health thresholds, and the effects are always the same. Level 1 : Slow Audible heartbeat, normal vision Level 2 : Fast audible heartbeat, blurred vision This is why a few alternative percentages are proposed, for those who prefer to be warned earlier or later.
As a rule, it is possible to keep only one threshold (no visual effect OR fast heartbeat + troubled vision) by adjusting the two values to the same level. Use SSEEdit, the Creation Kit or TESVSnip to quickly edit the numeric value. (This very small mod modifies only 2 variables of gameplay.) My custom setting is 45/25%.

So, in my assembly I'll turn OFF this mod, cos of many many many other UI incompabilities for it.

In Vanilla, by the time I hear the heartbeat I'm one hit away from dying, and I ALWAYS do.
Any options to have the blurred vision only happen when the health is critical?
I.E.
