This one was caused by saving the game while mounted on your horse. As a result of that, when you loaded the game, Skyrim got confused and decided you were your horse. If you save the game while not mounted, but standing next to your horse, and then go into third person you'll see that the third person camera is anchored to your horse rather than your character.
I got this months ago. I looked for ages, and the only solution to this one is not have it in the first place. But "player.tc" twice in the console solves the problem when loading games anyway, so it's not as big as some of the other horrible stuff this game does to saves sometimes.
Yeah I realised the horse thing the other day. Weirdly enough if I save while mounted on my horse now I don't have to enter the command and my camera works fine, for a while at least.
