   So if I use the spell on a random NPC they permanently become a werewolf?

      Yes. Another spell turns them permanently in dogs. You can spend your day turning Balgruuf from dog to werewolf and nobody would do nothing about it. And of course, CHEESE FOR EVERYONE!
