I like the look, and will most likely use this mod, but I do have to make one comment (sorry):
Sheogorath is hinted at being the Hero of Kvath in his dialog when he mentions that he had a personal involvement in the Oblivion Crisis. This is most likely because the choice from the TESIV: The Shivering Isles was defaulted to you having chosen to release Jyggalag from his curse, turing you into the new daedric prince of madness.
Could you mess with his eyes a bit? Maybe just make the pupils narrower? Or just plain blind? He doesn't look insane anymore... (EDIT: I'm still not going to install... I like Old Man Sheo.)
Yes his outfit could be changed. BU remember it is not the same Sheogorath! In Shivering Isles (SPOILERS) The character becomes Sheogorath. So (If I recall correctly) the God of Orders return will warrant a new Sheogorath every time.
Last time I checked, didn't Sheogorath say, during SI, that you would become him.....but not literally? But at least close enough to fight Jyggalag?
A lot of people seem to miss that. Or just conveniently ignore it.

The hero is basically someone to watch over the Shivering Isles and nothing more.

Although the wiki also hints at the whole hero of Kvatch thing, which contradicts the info on the page.
Just to add my two cents about Skyrim Sheogorath being the COC: - After defeating Jyggalag he first calls you mortal than corrects himself "King? God? It remains to be seen." Hinting that you may truly become him. - EVERYONE on the Isles recognizes you as the "new" Sheogorath, even that dolt Sheldon. You're even able to understand the gibberish one of the Bliss residents is saying, which before defeating Jyggalag was only able through an item Big Head gives you. - Bethesda confirmed that the COC is the new Sheo and they Wabbajacked themself to look like they did in Skyrim.
While I do like your variation of Uncle Sheo, I changed him with CK to my liking. Which is closer to the previous one but still keeping the faceof the "new" one.
