You are probably using AngeLydia mod and that .esp file that comes with it.
Here is the thing, you don't have to use that .esp file that comes with AngeLydia to have Lydia look like Angelina Jolie. It will only make Lydia wear some guards armor and alter her stats a bit, so if you want to use my mod - just turn off that one.
But don't worry, Lydia will still look like Angelina Jolie, I'm using that texture\mesh mod as well.

Ah, I know why.
You have two mods conflicted with each other.
You are probably using AngeLydia mod and that .esp file that comes with it.
