Would be nice to know which recipes it adds, maybe include it in a readme as well. Simpler ones like Restore/Fortify/Damage Health/Stamina/Magicka are easy enough to discover / remember on your own, but could consist of like half of the 23 included.
Both comments are worth thinking. The recipes could add some simple multi-effect poisons, like the slow poison: Deathbell + river betty. Deals a lot of poison damage and slows down the target. Useful when you want to rain some arrows at someone and stay away.

I have to say that this book is fantastic, a great way to incorporate a list of console codes in a lore friendly fashion.
One thing I noticed your book is missing, and that is "Leeks: 000669a5"
Is there a mod that will show all of the possible potions and the required ingredients?
While a big book of recipes is a little cheaty, there are recipes available to be found and bought throughout the vanilla game, too.
I'm thinking of other in-game guides, maybe item codes, console commands, etc.
