Just finished a play-through and have answers for some of the questions below.
If you're having trouble starting the quest/can't seem to get any unique dialogue, try saving inside The Grotto, closing Skyrim, and reloading the save. That fixed the dialogue glitch for me: you need to talk to the front guard with the battle axe to start the quest.
If you killed an opponent but are stuck in the arena because the quest didn't update, you probably used a console command to kill him/her. You have to physically kill your opponent for the quest to proceed. DO NOT loot your opponent's body because it will force teleport you back to the Bloodworks and break the quest.
All in all this mod was kind of painful to complete. Dialogue was lackluster and pointless, the quests were pretty short, and there was zero atmosphere inside the Arena. I didn't even feel like a Grand Champion, just a dude who killed other dudes in an empty room. The constant dialogue bugs pretty much render this mod a dud in my opinion: I could exploit a glitch where the organizer would keep giving me gold by exiting and re-entering the conversation. Unfortunately cannot endorse this mod.
The first thing that stood out was that the developer can't even spell. The second thing that stood out was that the developer can't figure out how to add torches to a room?
Very dull and disappointing mod. The dialogue scrolled so fast I couldn't read it. Short and absurdly easy fights, even on master or legendary difficulty. I did like some of the designs, and I liked the simplicity of the arena.
Overall boring and pointless. Rewards isn't even worth the effort.
I thought i should let you know that The Champion of Cyrodil never died, they are the current Sheogorath, I was just thinking you could just make their tomb empty
get the pit fighters mod in the steam workshop, it uses the previously unused windhelm arena cell and has no bugs like that and works properly. Even has an add on that lets you fight in other provinces.
try dwemer Colosseum ... it's the only arena mod that's 100 percent like the oblivion arena... but it has its bugs... nothing that would make you delete it though...

The worst mod I've ever used. Deleting.
