I just wanted to share my solution to really keep the mouse off the screen.
The trick is before you hit your hotkey for the alt-tab open the game menu up [esc] or [start] then hit the hotkey to quickly switch in out of the game. Once your back in the game just close the game menu and let the good times roll.
So to sum up, the trick is to not move your mouse at all when in the game menu after you hit your hotkey.
Hope I didn't waste anyones time, just wanted to share this technique that I use and works very easily and smoothly.
This works amazingly well! But is it just me or do I have to run the .exe every time I reboot my computer? Is there a way to get it to auto run when I start Skyrim?

I spend a lot of time bouncing between windows and I like having full-screen, thanks Guru82. I do get random switches however. Can happen anytime, without touching a key?

Xbox 360 controller support. Now you can switch to desktop with Left Bumper + Back and switch back to Skyrim with Left Bumper + Start
