It's one of the quests for the Mages Guild You don't have to wait for the "Necromancer's Moon" event to complete it but if You do wait You will see a cool scene of Necromancers performing a ritual to create black soul gems. You can also create black soul gems Yourself by placing empty grand sould gem on the altar and casting soul trap magic on it during that event

Yeah, you're right about Fort Linchal. I/we also found another altar in the area of Fort Wendelbek (at the Panther River), and as far as I know there is a 4th one in the area of Fort Istirius (between Skingrad and Kvatch).
I was never on such a quest, but I remember that one night we found such an altar - by pure coincidence, because of its purple beam of light. (Dark Fissure, near Cheydinhal)
