Works only halfway for me. I get the increased carry limit and (I think) speed, but I'm not slowing down as I pick up stuff. Right at my limit I'm as fast as if I was carrying nothing at all.
I went through the whole OpenMW Total Overhaul list, including setting up load order and running Delta Plugin.
For openMW users / the openMW client, there is no difference between a .esp file and .omwaddon file.
OpenMW knows this, and its interface reports everything as normal. The problem people run into is mod managers such as MO2 not knowing an omwaddon file is valid.
In the for instance of MO2, you can tell it to "install anyway" when it reports there are no valid files in an archive. You need to know how to activate files inside of the OpenMW launcher UI, though. If you're using other mod managers, you may just have to directly manually install omwaddon files.
Stamina drain felt a bit high even with 100 on each of the governing stats for stamina, changed fFatigueRunMult down to 6 and felt much better balanced. Other than that little problem for me this is amazing, much better than original Morrowinds movement speed of running through cold molasses

Am I supposed to be able to still run after stamina is depleted?
edit- I discovered Morrowind Better Running and placed it just before this mod. Now I enjoy CarryOn's benefits with a stamina that lasts for a long time but still makes me collapse when I run out.
Nevermind. I think combining them is making the menu effy. inventory is not displaying worn clothes/armor... maybe something to do with the settings ring from Morrowind Better Running.
