Okay, so, first off, I'm a transgender person IRL so I do understand the desire to genderflip characters or add transgender characters to fiction. I do it myself.
Buuuuutttt...
You cannot call this mod lore friendly. Macreedy is an established character within the Fallout franchise, he is seen as a nearly-teenage child in Fallout 3, has speaking lines, and is a player in the main quest. Could he be trans? Sure. Would he be able to look like a smoke hot babe in a world without civilization? No. The surgical face changing in game has always been a player convenience... But since the Railroad doe sit for synths all the time, it's clearly a part of the setting. You'd think that this means you coudl ahve a character get a full SRS preformed, but, no.
These booths do not let you, the player, change sex. (At least they didn't when I played Fallout 4 last. Mabey they updated the game to let you do it?) but they CERTAINLY do not have the capabilities to do an SRS operation in the Capital Wasteland, where Macreedy is from and also where the only doctor who can preform a facial reconstruction (no sex change, just a face change), is hidden away in a deathtrap of a ruin that nothing at all tells you about, you need to look it up online.
Whats more, these operations would be absurdly expensive. And it has to be operations. Drugs expire. There's no way Macreedy could have taken hormone therapy. The drugs for it would have expired 190 years ago. What's more Macreedy has been in the Commonwealth for not a long time at all, and all of the facial surgery booths are expencive to use. 1000s of caps. Now recall how the game treats 150 caps as a huge unthinkable sum that can keep a person fed and sheltered for months. ANd recall that Macreedy is sending money and stuff home to his son who is sick.
There's no no lore friendly way for Macreedy to look like this as a trans woman. If you want her too, great, more power too you! But please don't call it lore friendly. It's not. Lore friendly would have her at best looking like a femmy twink of a guy.
first of all, the commonwealth have a LOT of 200+ years old drugs who are in "perfect conditions", and still the fact that they have the tecnology to do a FACIAL SURGERY in the middle of a wasteland, is enogh to think that probably somewhere is a person who does SRS, and like ffs is fallout logic, they have fucking mini-nukes, AIs in robots that are advanced enough to be equal to a human mind without even invented the transistors, and f*#@, it is assumed that the fusion energy in fallout lore is "unlimited" and then ingame the fusion cores give u 15 fuckin minutes, come on.
and as far as i now, its not as if she said "i had SRS" (U dont need SRS to be Trans SMH), yeah, cbbe gives her a pussy, but u know, cbbe isnt exactly "lore-friendly", no human being would have such a perfect figure in a post apocalyptic environment...
SO, as a trans girl i can say: if you are trans, just enjoy the representation, it isnt such a big problem that there is a trans person existing in FO4, its just a mod wtf.

I kinda get where you're comin from, but you seem to be forgetting a few things about Fallout:
Autodoc's exist. If you can reconstruct a face, you can likely, with some experimentation, perform SRS (it's likely not the first time they've been patching up something besides a face, mind you).
One major part of the reason the player cannot change sex, is breaking game functionality. Once you're out of the vault as your chosen 'sex' (/'gender'), that's it. It's 'locked in'. Bethesda do some shenanigans in the pre-war scene in order to let you swap back and forth at all, one nice demonstration of just how 'weird' the pre-war scene is, is skeleton fuckery if you use say, ZeX and go through some/all of the pre-war scene as Nate, or pick Nora and use the console to 'undress'.
Oh and this is the same world in which people are making their own Jet, stimpaks/Med-X, etc. (speaking of, shouldn't that have all expired/been lost to time, too?) If the Institute can create pretty much physically identical synths to someone, I'm sure someone, somewhere can produce or simulate human growth hormones in sufficient quantities, consistently, to supply to one or more trans individuals.
I'm not really a fan of people calling s#*! "lore friendly" anyway, but this can be. Potentially.

Just installed this one with the voice mod from hatchet47. Looks great and works fine. Can you make this mod (and your other makeovers) work with Unique followers from FGmodule? Then every follower can have a unique body too (maybe I find a green one for Strong somewhere).
Glad to hear... I started some fiddling with a Unique Followers patch, but can't get it to work... yet. There is a "green skin" mod out there, but haven't heard whether it is compatible with my version of Strong. Jina

I think the strangest part about this is how much weight MacCready has put on. I mean, reg macCready looks like he eats once a week. That's plastic surgery for ya.
LOL as a transgender myself I find this amazing and actually makes sense keeping the same vocals because male to females don't have a change in pitch when they transition. On top of that Macreedy looks cute :3
