I've got the same problem. I finished the Blood Ties quest and got the Hematophage perk from Vance. All I get is a "..." from Brianna when I try to talk to her.
Same Issue here but I'm also using better family to make them look more vampire and a collections mod setup so my trouble could be just about anything.
My solution was to just load a save file before I finished the Blood Ties quest, used a console command to give me the vampire perk, and then talk to Brianna.
This used to work perfectly. Now it doesn't. The file hasn't changed so something is interfering with it, and that something seems to be the Unofficial Fallout 3 Patch.esm. I've always played with that so I assume that a later version introduced a bug. In fact, since starting a new game recently I've noticed a lot of things not working quite right - like hacking terminals. Finding an earlier version might solve this and other things.
EDIT: Confirmed the latest UUF3P breaks this mod. Most likely others. The mod still works properly with version 3.1.7. I didn't test all the versions between that and the latest so there might be a later one.
I'm having an issue where i pass the Hematophage check, but she dosen't follow me. And when i try to talk to her again, the check is still there. Any idea on how i can fix this?
Had to cheat this one, a bit. Maybe it's thanks to an odd combination of mods, but I was able to finish Blood Ties without ever negotiating a deal between Arafu and Vance. Without have done that negotiation, there was no option to ask to become a Hematophage perk. I ended up having to use the console to add the perk, but after doing that, I was able to hire Brianna. I've just started, so no opinion, yet, on how her quests work.
Successfully hired Brianna tonight and did the first few of her quests. No issues at all, fights great, and the quest line is enjoyable. Can't wait to do more. Endorsed. :)

Have successfully completed the "Family" quests but am still not able to hire Brianna, I only get "Hiya Kid" dialogue from her now that the quest is complete. Have moved the esp to the lowest position in the load order and no change. Anyone have this problem or know what can be causing it? Looks like it's a great mod, would hate to not get to use it.
Did you read the description? You need either the cannabal perk or the vampire perk you get from Vance to hire her. If you have neither then you cant hire her.
The description states "She can be hired at any time with either the Cannibalism or Hematophage perk, but heres the fun part, you get to decide"... This doesn't make it all clear that it's actually the PlayerCharacter that needs either... I was just having the same issue, so thanks for clearing that up! ^_^
So, after I set her new home as my Megaton house a red square with a large exclamation mark appears and covers her character model, and because o this sometimes her hair disappears for a little bit and she is bald. Any idea how I can fix this?

The only mod companion I have recruited so far is Brianna, 'nif that affects this.
The reason Brianna cowers and flees is because you set her tactics to use ranged weapons but only gave her melee weapons. If you want her to harvest blood you must set her tactics to MELEE.
I have tested this with custom mod swords and knives and she will harvest blood if you have set her tactics to melee. In fact, she even harvested blood from giant ants.
EDIT: She will even harvest blood while using a Supersledge Hammer.

EDIT: Confirmed the latest UUF3P breaks this mod. Most likely others. The mod still works properly with version 3.1.7. I didn't test all the versions between that and the latest so there might be a later one.

The only mod companion I have recruited so far is Brianna, 'nif that affects this.
If you want her to harvest blood you must set her tactics to MELEE.
I have tested this with custom mod swords and knives and she will harvest blood if you have set her tactics to melee.
In fact, she even harvested blood from giant ants.
EDIT: She will even harvest blood while using a Supersledge Hammer.
