Extract only "SpecialK64.dll" to game installation directory
Rename "SpecialK64.dll" to "dxgi.dll" (updated on 7/11/2021 for ReShade compatibility)

Extract "SK_Res" folder to game installation directory
Done
Install with ReShade (after Install) (I skipped steps 1, 2, and 12) (on step 15 I had to set it to "early")
