Save NG + lvl 100 with JUST main mission completed. I have all itens. I did not do the secondary quests . I did not do any exploration. I did not do Hearts of Stone and Blood and Wine just started to release Toussaint.

This is a Save NG + lvl 100 with all items. It has only complete main mission.
After completing all the imaginable endings I ended up getting a little tired of the main quest. I decided to be a free witcher! So, I did my last gameplay focusing on the main quest, only doing the essentials and leaving the rest for later.
I always found it terrible to have only one NG + available, so I did this Savegame to play endlessly, always returning it when I finish the game.
Have some money.
It has all the elixirs, potions and bombs.
You have all the witcher Grand Master sets in the chest.
I did not do the secondary quests (only the essential ones, like Dr. Moreau's to get the new mutations and the Drakenborg, to free the enchantress of runes).
I did not do any exploration.
I did not do Hearts of Stone.
Blood and Wine just started, to release the map of Toussaint.

P.S.: Just for curiosity, in the main quest I killed Radovid, Ciri became empress and stayed with Triss. Of all for me was the most satisfactory setting!...
