Better Dying Grandpa replaces the art at the very beginning of the game with something slightly more well drawn. Unfortunately, Grandpa's fire will no longer have four frames of animation which some may consider a game breaker, but on the plus side Grandpa's deathbed is no longer a cot with mismatched legs.
