Please note that the outfits only work with Fashion Sense items not clothing made in game!

In the GMCM Farmer 2.0 Looks is where you will find the outfit names, this is also where you can rename them to make your life easier.

You create the outfits by loading up a save (i use a trash one to set up the outfits and then share them with all of my saves), and grabbing your fashion sense mirror: you can buy this from Pierre or choose to start in game with it. When you go into the mirror you should see a little dresser icon on the side, this is where you will create the outfits according to the name in the GMCM!
Then your outfits should load with your respective portraits.

Please note if you do not want to use any of the outfits options you do not have to, just turn them off in the GMCM. The entire mod is highly configurable so if there is anything you do not want to use just simply turn it off.
