   When using this mod with Underwear.dll, does this become the only underwear distributed, or is it a mix of this and the other underwear?

      This becomes the only underwear distributed.
      
      Check out N.U.D.E. powered by Underwear.dll if you want many different underwear distributed.

   you use a skin texture mod for your characters or just vanilla skin?

      If you're talking about the characters in the mod's picture, those are from Sleep or Swim Attire, you'd have to ask the creator.
