The Dragon Cult - Priesthood doesn't require a new game and it's only compatible with latest version of Custom Skills Framework.
It requires B.E.E.S. if you are not running the game version 1.6.1130+.

When you interact with the altar in Bromjunaar Sanctuary, which you can find by exploring Labyrinthian, you will be asked to join the Dragon Cult. If you do so, you will receive a Lesser Power called Kneel. Using it will force your character to kneel down and will grant you access to the Priesthood perk tree.
You can acquire new perks for Priesthood by killing Draugr while wearing a Dragon Priest mask.
In accepting the Priesthood, you will be cursed with Dragon Binding, which increases your Shout cooldown by 50%. Dragon Binding can be removed by unlocking all Priesthood perks.

Shadow Legacy (2/2): Your Shouts summon a Wyrm\an additional Wyrm for 60 seconds.*
Ethereal Guard (2/2): Your Wyrms gain 150/300 Armor Rating and 25/50% Magic Resistance.
Elemental Curse: Wyrms reduce the Resistance of all nearby enemies by 25%.
Dragon’s Grasp (2/2): Your Wyrms deal 50/100% more damage.
Ancient Wisdom (2/2): Staff and weapon enchantments cost 10/25% less while wearing a Dragon Priest mask.
Arcane Mastery (2/2): Staff enchantments are 25/50% stronger while wearing a Dragon Priest mask.
Dragon’s Embrace: Weapon enchantments are 50% stronger while wearing a Dragon Priest mask.
Reflective Aura (2/2): You reflect 50/100% of incoming melee damage back at your attacker while wearing robes.
Scaled Armor (2/2): Your Armor Rating is increased by 50/100 while wearing robes.
Dragon's Veil: Your Fire, Frost and Shock Resistance is increased by 25% while wearing robes.
Unbind: Dragon Binding effect is removed.
*Wyrms take the aggro of nearby enemies and can only be summoned using Fire and Frost Breath. If Stormcrown is installed, you can also summon them by using Lightning and Poison Breath. The duration can be increased with Sky Above perk from Stormcrown and Fortify Shout Duration enchantment from Thaumaturgy.

Priesthood is also completely optional, so if you are only interested to Konahrik, you will be able to place every Dragon Priest mask without being forced to accept.
Compatibility
The Dragon Cult - Priesthood does not contain any edit to vanilla records, therefore it should not have any compatibility issue. Priesthood is also compatible out of the box with Belt-Worn Dragon Priest Masks, Dragon Priest Hood Variants, King-Priest, Konahrik's Accoutrements, Legacy of the Dragonborn, and Wyrmstooth, but it's not intended to work with unenchanted and player crafted masks.
