Judging how Synthesis patcher works in this regards I would say yes. The patcher seems to only forward the NPCs records from AI Overhaul for max compatibility while this patch do that (for ICOW) and edit Idle markers in the cell. I don't think the Synthesis Patcher for AI Overhaul edit cell markers. So I would suggest placing this patch below both AI Overhaul & ICOW, then run the synthesis patcher to forward the npc records.
Good news is this patch does what it says it does and more; not only did it fix the NPC idle markers, it also fixed the dark face bug I was having with several college NPCs, including Phinis and Arniel. Bad news is, though I'm not entirely sure it has to do with this patch, that when I loaded up my save, a couple of the college NPCs were naked, including Faralda and Arniel. They were fully clothed before installing this mod, so I can only assume this patch caused that to happen. I fixed it for now by using the reset inventory command on them, but this might want to be looked into.
This usually happens when the actual outfit of the NPC is modified, which can be done by a number of other mods. GrantyBoy recommends using resetinventory if you come across this in your game iirc (I know I've had to use it a couple times myself). I'll take a look soon, I have an idea of what the issue might be but not 100% sure.
This is indeed an issue of the outfit being changed on a game in progress. What happened was, when you installed the patch, it gave the NPC's the different outfits from ICOW. It is totally safe (and recommended) to do RESETINVENTORY on the naked NPC's. On a new game, this would not happen.
