This has always and will ever be related to load order and install order of your mods. Make sure that Simple Children is installed last and FaceGenForKids.esp is loaded low in your load order. If some other mod edits the kids in any way, they will overwrite the faces provided in this mod, causing dark faces.
I will reiterate that there are many patches located in the Optional section of the Downloads, and off-page patches are listed in the Mod Specific Compatibility section of the Description.
I've tried to make the description as concise and straightforward as possible, so please read through it carefully.

Do not install the mesh patch for Child NPC Mods in the installer. I believe that was meant for RS Children only. With simple children the mesh is broken and causes issues, the most noticeable being child's with bald heads, the worst (I've seen) being exploded polygons or broken mesh glitch.
Without the mesh patch installed Simple Children and Wet and Cold work perfectly fine.
Okay, I know that I'm sounding like a broken record, but could someone PLEASE spell this out step by step? Because I still can't figure it out.
I followed every instruction: only left the *tri files of TK Children, downloaded Simple Children, deleted the FaceGenForKids.esp, downloaded the new one for the new version of USSEP that's in the comments, and just put that in my Data folder.
I even went into the "textures" folder in my Skyrim Data folder, found a 'TK Children' folder and deleted it to be sure that I had ONLY *tri files left.
When that didn't work, I uninstalled the mods and then installed everything manually, but my game isn't loading any manually downloaded mods. I don't like downloading manually and I usually use Vortex, but I decided to try it because nothing else was working.
But no matter what I do, every time I go back into the game, the kids have dark, bald, TK faces. What the heck am I doing wrong?
I've pre-prepared the STEP Modifications patch to save people trouble and time. The short version is, this mod was written for an old version of USSEP, and has an "Error: could not be resolved" problem in it that causes crashes. Only takes a moment to fix it in xEdit, but not everyone is comfortable doing that, so I've zipped up the patched version of FacegenForKids.esp for you here:
Making my first comment just to say thank you, man. Even after going into xedit and following directions to fix stuff myself, my game STILL hit an infinite load, until I used both the patches you posted. Works like a dream now. Thanks for helping newbies like me! We're hopeless XD
Can someone explain what exactly I need to do to TK Children? I use Vortex, downloaded the mod, installed and deleted everything in the folder but the .tri files, but nothing change in the children, only the dark faces.
I'm on MO2 and I also have no idea what he meant by that installation. Do we simply install the TK mod but leave the esps unticked? Or do we need to go into the folder and literally delete everything one by one except those tri files?
Dentro do TK tem uma pasta com alguns arquivos .tri, a pasta CHARACTER ASSETS, deixa essa pasta e apaga todas as outras, então instala o TK só com essa pasta, depois instala o Simple Chindren
You do literally just what is said in the description.
As repeated by ambuzi below; "Inside TK there is a folder with some .tri files, the CHARACTER ASSETS folder, leave that folder and delete all the others, then install TK with just that folder, then install Simple Children"
If you don't know how to search for a specific file extension in explorer, * followed by the extension (example; *.tri will filter just .tri files).
So do you leave the file structure intact TKChildrenSE/00 Main/Meshes/Actors/Character/Character assets and install the zip file or do you literally pull the character asset folder and its single sub folder and paste that into Data/Meshes/Actors/Character/character assets in your game?
So the confusion comes from wondering if the word "install" is being used literally or figuratively since that literally changes the meaning of what you are supposed to do depending on how fast and loose the mod author is with the nomenclature.
Hi, I know I'm a bit late for the party but I have a strange bug where all the children have grey eyes ? Not like there is no texture or anything, but just... Grey. Did they all turned into werewolves in the process and what can I do to fix their eyes ? Thanks !
Love the mod. Wish the other users could use it, but I honestly had no problems with it.
Just a heads up (for those of you who can get the mod working), the Immersive Weapons Integration patch, and I assume others as well, forward the same, old USSEP record that causes infinite load screen. Make the same changes to it you did for the original mod in SSEEdit.
For a mod called Simple Children, trying to get it to work is anything but simple. I cannot, in it's current state, recommend this mod (for whatever that's worth)
So, I have given up trying to understand what's causing my ILS issue with this mod. I'm sure it's something I'm doing, I just don't know what. I had this mod installed for a couple weeks, no problem. Then yesterday I installed a handful of mods (see below).
Something in TK Children/Simple Children and assorted patches and those mods does not play nice. I cannot get my game to work with those mods installed, and TK/Simple Children installed. I get an ILS at the main menu (no menu pops up). I do not understand modding nearly well enough to figure this out on my own. The only I'm posting this is because I figured it should probably be an issue people are aware of, in case someone/the mod author wants to take a gander. This isn't a complaint or an accusation, this is just me saying "hey there's a conflict here somewhere". It's not even just this mod, I had a black face/no clothes issue with Babette using RS Children. (although, I'm apparently not the only one, according to the comments on that mod.)
Specifically, I installed some mods, had no idea why I was getting ILS (suspected a botched DynDOLOD installation, actually), spent hours toggling everything on and off, finally discovered that my mod list worked perfect as long as TK/Simple children wasn't installed. Uninstalled it, switched to RS Children, immediately encountered black face/no clothes Babette. Re-installed this mod (including Darklocq's Facegen patch below), and my ILS issue popped right back up.
Outside of my problematic mod list, this mod is amazing. Comprehensive patches and well-done overhaul. All in one place, without the massive headache that seems to accompany every other children's mod on the nexus.
For those who want a detailed report, I use MO2 and have these particular related files installed: (yes, I ran LOOT and have the .esps listed according to tetchystar's instructions)
TK Children (yes, I disabled/deleted the .esm and .esp) Simple Children Simple Children Updated Textures Simple Children Patches
And these are the mods I installed, after which I had the ILS issue:
Dark Brotherhood Reborn - Dawnstar Sanctuary Save the Dark Brotherhood 1.1 Your Choices Matter - Dark Brotherhood Expansion main file SSE Save the Dark Brotherhood Skyrim Special Edition - PATCH Recruit More Blades (Extra Blades Fixed and Cleaned) Skyhaven Modified Opulent Thieves Guild - FPS Fix Dark Brotherhood Reborn - Patches Tweaks and Enhancements Better Shadowmere (Default Version) Better Shadowmere - Saddle Mesh Replacer (Shadowmere Saddle - Vanilla)
If you want my full mod/esp/esl list, I'm happy to provide it, but I'm not posting it here because I don't want to take up the whole damn page.
Hello ! I have the exact same problem. I'm running a new mod list and have few mods installed, only textures for the moment. So I ran a few tests and found it worked fine without USSEP I just made a merged mod, mannually, between the required files (all .tri files) from TK children, and the main file from simple children added in second. It worked fine for the menu. The culprit is the ESP "facegenforkids" BUT without it, just dark faces for kids ! Soooo I don't know what else we can do about it, except using another mod, wich is really sad...
I had the same issue. Thought I broke my game and reinstalled it and kept having the issue until I deleted this mod and the files from the other mod. Shame because I don't like the Redguard Children mod (makes some of the kids' skin tones too dark) and this mod looked promising. Off to find something else.
Thanks for the link! I think that one is included in the FOMOD patch installer. "AAA_USSEP_FacegenForKids.esp" Again, I didn't have any issues with ILS until AFTER I installed the mods listed in post. So, I don't think this particular patch is the issue.

Includes a ReadMe (summary: install the original mod first, then replace the original FacegenForKids.esp with mine).
Hopefully the mod author will just update the original mod to include this so my patch can go away.
If you also use "Beyond Skyrim: Bruma", see my updated Simple Children patch for that, too (here, in a reply to a post by KotoriKaji).
Thanks
